أنا، ولي الأمر/الوصي القانوني الموقّع أدناه، أوافق على دخول ولدي/أولادي واللعب في كيدز هَب. وأفهم أن مشاركة ولدي/أولادي تكون على مسؤوليتي الخاصة، وأن أنشطة اللعب العادية قد ينتج عنها السقوط أو الاصطدام أو إصابات بسيطة. كما أفهم وأقر بأن المشاركة في أنشطة اللعب الداخلية تنطوي على مخاطر طبيعية وملازمة لهذا النوع من الأنشطة، وأتحمل طوعًا جميع المخاطر المرتبطة باستخدام ولدي/أولادي لمنطقة اللعب. وأوافق على الالتزام بجميع تعليمات وإرشادات السلامة. كما أمنح كيدز هَب الإذن باستخدام الصور ومقاطع الفيديو الخاصة بولدي/أولادي لأغراض ترويجية

وبموجب هذا، أُخلي مسؤولية كيدز هَب ومالكيها وإدارتها وموظفيها من أي مطالبات أو مسؤوليات ناتجة عن أي أضرار أو إصابات قد يتعرض لها ولدي/أولادي أثناء استخدام منطقة اللعب. وأقر بأنني قد قرأت وفهمت هذا الإقرار بإخلاء المسؤولية وأوافق على جميع شروطه، وأوقع عليه بمحض إرادتي واختياري.

I, the undersigned parent/legal guardian, give permission for my child(ren) to enter and play at KIDS HUB. I understand that my child(ren)'s participation is at my own risk and responsibility and that normal play activities may involve falls, collisions, and minor injuries. I understand and acknowledge that participation in indoor play activities involves inherent risks. I voluntarily assume all risks associated with my child(ren)'s use of the play area. I agree to follow all safety guidelines and instructions. I grant KIDS HUB permission to use photographs and videos of my child(ren) for promotional purposes.

I hereby release and discharge KIDS HUB, its owners, management and employees, from all liability and claims arising from any damages or injuries suffered by my child(ren) while using the play area. I have read and understood this Liability Waiver and agree to its terms. I am signing it freely and voluntarily.
