أنا، ولي الأمر/الوصي القانوني الموقّع أدناه، أوافق على دخول طفلي/أطفالي واللعب في كيدز هب (KIDS HUB). أُدرك أن مشاركة طفلي/أطفالي في أنشطة اللعب تكون على مسؤوليتي الخاصة، وأن أنشطة اللعب العادية قد تتضمن السقوط أو الاصطدام أو التعرض لإصابات بسيطة. كما أُقر بوجود مخاطر ملازمة لأنشطة اللعب، بما في ذلك على سبيل المثال لا الحصر الانزلاق والسقوط والاصطدام وغيرها من الإصابات، وأتحمل طواعيةً جميع المخاطر المرتبطة باستخدام طفلي/أطفالي لمنطقة اللعب.

أوافق على الالتزام بجميع إرشادات وتعليمات السلامة الصادرة من كيدز هب وموظفيه. وأُدرك أن عدم الالتزام بهذه التعليمات قد يزيد من احتمالية التعرض للإصابة. وبموجب هذا الإقرار، أُخلي مسؤولية كيدز هب ومُلّاكه وموظفيه من أي مطالبات أو مسؤوليات قد تنشأ عن أي إصابات أثناء اللعب.

أُقر بأنني قرأت هذا الإقرار وفهمت جميع شروطه وأحكامه بشكل كامل، وأوقع عليه بمحض إرادتي ودون أي إكراه. كما أُفوّض كيدز هب بالاحتفاظ بهذا الإقرار واستخدامه كإقرار متعدد الاستخدامات لزيارات طفلي/أطفالي المستقبلية، وذلك إلى حين قيامي بإلغائه خطياً.

I, the undersigned parent/legal guardian, give permission for my child(ren) to enter and play at KIDS HUB. I understand that my child(ren)'s participation is at my own risk and responsibility and that normal play activities may involve falls, collisions, and minor injuries. I acknowledge the inherent risk involved, including but not limited to slips, falls, collision and other injuries. I voluntarily assume all risks associated in playing in the play area.

I agree to follow all safety guidelines and instructions provided by KIDS HUB and its employees. I understand that failure to adhere to these guidelines may result in an increased risk of injury. I hereby release and discharge KIDS HUB, its employees, and owners from all liability, claims, demands, actions, arising out for any and all damages and injuries suffered by my child(ren) while playing in the play area.

I have read the liability waiver and fully understand its terms and conditions and I sign it freely and voluntarily. I hereby expressly authorize KIDS HUB to use this Waiver as a multi-use waiver until such time as I revoke it in writing.
